NP-PA-VNA Supportive Care Hospice - Req # 78495
Under direction of the Director of Supportive Care Services or designee, work in collaboration with a Physician, and other palliative care team members, in an active practice to deliver hospice care and related health care services to patients and families. Perform a wide variety of duties including but not limited to: bedside patient/family evaluation, assisting with diagnosis, treatment and management of acute and chronic health conditions, educating/counseling patients, families, and/or caregivers as to preventative care, medical problems, etc and serving as a nursing resource for education and consultation to members of the interdisciplinary team.Provide functional or technical supervision of assigned personnel within areas of own position responsibilities where scope of supervisory responsibility is limited to direction, guidance, explanation, etc.

Hours: 40 hours Mon thru Fri 8a-4:30p

Education and/or Experience Requirements:
Must be fully vaccinated against seasonal Influenza and the COVID 19 virus and provide required vaccination documentation unless approved for an exemption or deferral by Southcoast prior to starting employment.
